Indications
- Peptic ulcer diseases

- Ulcer induced by non-steroidal anti-inflammatory drugs (NSAIDs)
- Eradication of Helicobacter pylori (in combination with antibiotics)
- Zollinger-Ellison Syndrome
Pharmacology
- Proton pump inhibitor
- Suppression of the final step in gastric acid production
- Covalently binds to H+/K+ATPase enzyme system
- Inhibition of both basal and stimulated gastric acid secretion
- Effect persists longer than 24 hours
Dosage & Administration
- 40 mg daily in the morning for benign gastric ulcer for 4 weeks, continued for further 4 weeks if not fully healed
- 20-40 mg daily in the morning for gastro-esophageal reflux disease for 4 weeks, continued for further 4 weeks if not fully healed
- 40 mg daily in the morning for duodenal ulcer for 2 weeks, continued for further 2 weeks if not fully healed
- Pantoprazole is recommended at a dose of 40 mg twice daily in association with antimicrobial agents for duodenal ulcer associated with Helicobacter pylori
- 20 mg daily for prophylaxis of NSAID-associated gastric or duodenal ulcer
- Initially 80 mg once daily for Zollinger-Ellison Syndrome, adjusted according to response; daily doses above 80 mg given in 2 divided doses
- 40 mg IV once daily for duodenal ulcer and gastric ulcer for 7-10 days
- 40 mg IV once daily for gastroesophageal reflux disease associated with a history of erosive esophagitis for 7-10 days
- IV 80 mg for prevention of rebleeding in peptic ulcer, followed by 8 mg/hour infusion for 72 hours
- 80 mg IV for prophylaxis of acid aspiration every 12 hours for 24 hours, followed by 40mg every 12 hours
- 80 mg IV every 12 hours for long-term management of Zollinger-Ellison Syndrome and other pathological hypersecretory conditions
Interaction
No significant drug interactions observed in clinical studies
Contraindications
Contraindicated in patients with known hypersensitivity to any of the components of the formulation
Side Effects
- Well tolerated in both short-term and long-term treatment
- Common side effects: headache, diarrhea
- Rarely included side effects: abdominal pain, flatulence, rash, insomnia, hyperglycemia
Pregnancy & Lactation
- US FDA Pregnancy Category B
- No adequate and well-controlled studies in pregnant women
- Excretion in human milk
- Use during pregnancy only if clearly needed
- Decision should be made whether to discontinue nursing or drug, taking into account the benefit of the drug to the mother
Precautions & Warnings
- Pantogen tablet should not be split, chewed or crushed
- Long-term therapy may lead to malabsorption of cyanocobalamin (Vitamin B12)
- May increase the risk of osteoporosis related disorders
Overdose Effects
- No known symptoms of overdosage in humans
- Highly protein bound, not readily dialyzable
- Apart from symptomatic and supportive management, no specific therapy recommended

Reconstitution
- IV injection: Intravenous administration only, reconstituted with 0.9% Sodium Chloride Injection and given slowly over a period of at least 2 to 5 minutes
- IV infusion: Intravenous infusion over a period of approximately 15 minutes, reconstituted with 10 ml of 0.9% Sodium Chloride Injection and further diluted with 0.9% Sodium Chloride Injection or 5% Dextrose or Lactated Ringer's Injection to a final volume of 100 ml
- Reconstituted solution may be stored at room temperature for a maximum of 4 hours before further dilution, and admixed solution may be stored at room temperature and must be used within 24 hours from the time of initial reconstitution
