Pharmacology

Interferes with bacterial cell wall synthesis, penetrates bacterial cell walls, stable to serine beta-lactamases, affinity for Penicillin Binding Proteins, active against Gram-positive and Gram-negative bacteria

Dosage

  • Adults: 500 mg to 1 gm by intravenous administration every 8 hours
  • Children: 10 to 40 mg/kg intravenously every 8 hours
  • Intra-abdominal infections: 20 mg/kg every 8 hours
  • Cystic fibrosis (4-18 years): 25-40 mg/kg every 8 hours
  • Meningitis: 40 mg/kg IV every 8 hours
  • Febrile neutropenia: 20 mg/kg every 8 hours
  • Children over 50 kg weight: use adult dosage

Administration

Intravenous infusion over approximately 15-30 minutes or as intravenous bolus (5 to 20 ml) over approximately 3-5 minutes

Interaction

  • Probenecid competes with Merolab for active tubular secretion
  • May reduce serum valproic acid levels

Precautions & Warnings

Discontinue if allergic reaction occurs, careful monitoring in hepatic disease

Use in Special Populations

  • Dosage reduction in renal impairment
  • No dosage adjustments in hepatic impairment
  • Caution in infants under 3 months

Overdose Effects

Symptomatic treatment for accidental overdose, hemodialysis for renal impairment

Reconstitution

  • Intravenous bolus Administration: Reconstitute with sterile water for injection
