Indications
- Prophylactic agent for preventing ifosfamide-induced hemorrhagic cystitis
- Preventing high dose cyclophosphamide-induced hemorrhagic cystitis
Pharmacology
- Rapid oxidation to major metabolite, Mesna disulfide (diMesna)
- Elimination of Mesna disulfide by the kidneys
- Reduction of Mesna disulfide to free thiol compound, Mesna
- Chemical reaction with urotoxic ifosfamide metabolites
- Detoxification of acrolein and 4-hydroxy-ifosfamide
- Lack of interference with antitumor efficacy

- Fractionated dosing schedule for ifosfamide induced hemorrhagic cystitis
- Intravenous bolus injection as per ifosfamide dosage at specific time intervals

Precautions & Warnings
- Reported allergic reactions
- Possible higher incidence of allergic reactions in patients with autoimmune disorders
- No prevention or alleviation of other adverse reactions or toxicities associated with ifosfamide therapy
- Up to 6% of patients developed hematuria
- Requirement for daily examination of urine for hematuria
- Reduced effectiveness in preventing hemorrhagic cystitis in all patients
- Benzyl alcohol content caution in pediatric patients
