Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
- Reduction of diabetic complications in overweight type 2 diabetic adult patients
- Monotherapy for type 2 diabetes mellitus
- Combination therapy with other oral antidiabetic agents
- Combination therapy with insulin
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers both basal and postprandial plasma glucose
- Increases insulin sensitivity
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
Dosage & Administration
- Dosage must be individualized based on effectiveness and tolerance
- Usual starting dose for adults: 500 mg twice a day
- Usual starting dose for children: 500 mg twice a day
- Maximum daily dose for adults: 2000 mg
- Maximum daily dose for children: 2000 mg
- Metformin extended release tablet should be swallowed whole
Interaction
- Enhances absorption of Nifedipine
- Minimal effects on Nifedipine
- Potential interaction with cationic drugs
- No effect on Cimetidine pharmacokinetics
- Certain drugs may produce hyperglycemia
Contraindications
- Hypersensitivity to the active substance or any excipients
- Any type of acute metabolic acidosis
- Severe renal failure (GFR < 30 mL/min)
- Acute conditions with potential to alter renal function
- Acute or chronic diseases causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease in vitamin B12 absorption and serum levels
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders such as nausea, vomiting, and diarrhea
- Liver function test abnormalities or hepatitis
- Skin reactions such as erythema and urticaria
Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy is associated with increased risk of congenital abnormalities
- Insulin recommended for diabetes treatment during pregnancy
- Metformin excreted into human breast milk
- No adverse effects observed in breastfed newborns/infants
Precautions & Warnings
- Substantially excreted by the kidney
- Risk of lactic acidosis increases with impaired renal function
- Lowers vitamin B12 levels
- Increases risk of hypoglycemia when used with insulin
- Regular monitoring of renal function advised
- Therapy should be stopped before surgery and reinstated based on renal function
Use in Special Populations
- Elderly: dosage adjustment based on renal function
- Pediatric: diagnosis confirmation before treatment initiation
- Regular assessment of renal function in all diabetics
- Particular caution in children aged between 10 and 12 years
Overdose Effects
- No hypoglycemia seen with doses up to 85 gm
- Lactic acidosis occurred in such circumstances
- Lactic acidosis is a medical emergency and must be treated in hospital
