Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
- Reduction of diabetic complications in overweight type 2 diabetic adult patients
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers both basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity
Dosage & Administration
- Individualized dosage based on effectiveness and tolerance
- Usual starting dose for adults: 500 mg twice a day or 850 mg once a day
- Usual starting dose for children: 500 mg twice a day
- Dosage increases should be made in increments of 500 mg or 850 mg
- Maximum recommended daily dose for adults: 2000 mg per day
- Maximum recommended daily dose for children: 2000 mg per day
- Dosage for renal impaired patients
- Usual starting dose for Metformin XR: 500 mg once daily with the evening meal
- Maximum recommended daily dose for Metformin XR: 2000 mg once daily with the evening meal
- Swallow Metformin XR tablet whole and never crush, cut or chew
Interaction
- No information available about the interaction of Benformin and Furosemide when co-administered chronically
- Nifedipine appears to enhance the absorption of Benformin
- Benformin had minimal effects on Nifedipine
- Possible interaction with cationic drugs eliminated by renal tubular secretion
- Benformin has no effect on Cimetidine pharmacokinetics
- Certain drugs tend to produce hyperglycemia and may lead to loss of glycemic control
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Any type of acute metabolic acidosis
- Severe renal failure
- Acute conditions with potential to alter renal function
- Acute or chronic diseases which may cause tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite
- Liver function test abnormalities or hepatitis
- Skin reactions like erythema, pruritus, urticaria
Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities and perinatal mortality
- Breastfeeding not recommended during metformin treatment due to limited data available
Precautions & Warnings
- Substantially excreted by the kidney
- Increases risk of hypoglycemia when used in combination with insulin or insulin secretagogue
- Regular monitoring of renal function advised
- Benformin therapy should be stopped before surgery and reinstated only after control of renal function
- Caution when prescribing to children aged between 10 and 12 years
- Regular assessment of renal function is necessary in elderly subjects
Use in Special Populations
- Dosage adjustment based on renal function in elderly subjects
- No effect on growth and puberty detected during controlled clinical studies
- Careful follow-up of the effect of metformin on growth and puberty recommended in children
- Renal function assessment before treatment initiation and regularly thereafter
Overdose Effects
- Hypoglycemia not seen with Benformin doses up to 85 gm
- Lactic acidosis is a medical emergency and must be treated in hospital
- Hemodialysis is the most effective method to remove lactate and Benformin
