Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
- Reduction of diabetic complications in overweight patients treated with metformin as first-line therapy after diet failure
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers both basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity
Dosage & Administration
- Dosage must be individualized based on effectiveness and tolerance
- Maximum recommended daily doses should not be exceeded
- Dosage increases should be made in increments of 500 mg weekly
- Dosage increases should be made in increments of 850 mg every 2 weeks
- Metformin immediate release tablet
- Metformin extended release tablet
- Dosage adjustments for renal impaired patient
Interaction
- Co-administration with Furosemide
- Co-administration with Nifedipine
- Potential interaction with cationic drugs
- Effects on Cimetidine pharmacokinetics

Contraindications
- Hypersensitivity to the active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Acute conditions with potential to alter renal function
- Acute or chronic diseases causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders
- Liver function test abnormalities
Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy
- Increased risk of congenital abnormalities and perinatal mortality
- Excretion into human breast milk
- No adverse effects observed in breastfed newborns/infants
- Limited data available on breastfeeding
Precautions & Warnings
- Substantially excreted by the kidney
- Risk of Benformin accumulation and lactic acidosis
- Lowering of vitamin B12 level
- Increased risk of hypoglycemia in combination with insulin
- Regular monitoring of renal function
- Stopping Benformin before surgery
- Annual estimation of Vitamin B-12 levels
- Monitoring blood glucose during combined therapy with a sulphonylurea
- Interaction with anticoagulants
Use in Special Populations
- Adjustment of metformin dosage based on renal function in elderly subjects
- Regular assessment of renal function in elderly subjects
- Caution in prescribing to children aged between 10 and 12 years
- Assessment of creatinine clearance before treatment initiation
- Regular GFR assessment before treatment initiation and thereafter
- Caution in situations where renal function may become impaired
Overdose Effects
- Hypoglycemia not seen with Benformin doses up to 85 gm
- Occurrence of lactic acidosis in high doses
- Lactic acidosis is a medical emergency
- Hemodialysis as the most effective method for removal
