Function

Dosage & Administration

  • Usual starting dose of 500mg twice a day for adults

  • Maximum daily dose of 2000mg for adults
  • Swallow extended release tablet whole and never crush, cut or chew
  • Special precaution for renal impaired patients

Interaction

  • Enhancement of absorption with Nifedipine
  • Potential interaction with cationic drugs eliminated by renal tubular secretion
  • Effects on Cimetidine pharmacokinetics
  • Certain drugs tend to produce hyperglycemia and may lead to loss of glycemic control

Contraindications

  • Hypersensitivity to the active substance or to any of the excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Acute conditions altering renal function
  • Acute or chronic diseases causing tissue hypoxia
  • Hepatic insufficiency
  • Alcohol intoxication or alcoholism

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Peripheral neuropathy in patients with vitamin B12 deficiency
  • Taste disturbance
  • Encephalopathy
  • Gastrointestinal disorders like nausea, vomiting, diarrhea, abdominal pain, loss of appetite
  • Liver function test abnormalities or hepatitis
  • Skin reactions like erythema, pruritus, urticaria

Pregnancy & Lactation

  • Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
  • Excretion into human breast milk
  • Breastfeeding not recommended during metformin treatment

Precautions & Warnings

  • Substantial excretion by the kidney
  • Lowering of vitamin B12 level
  • Increased risk of hypoglycemia when used in combination with insulin or insulin secretagogue
  • Regular monitoring of renal function advised
  • Therapy cessation before certain medical procedures
  • Annual estimation of Vitamin B-12 levels for continuous therapy patients
  • Monitoring for hypoglycemia during combined sulphonylurea therapy
  • Caution during cimetidine therapy
  • Potential interaction with anticoagulants

Use in Special Populations

  • Dosage adjustment based on renal function in elderly subjects
  • Confirmation of type 2 diabetes mellitus before treatment initiation in pediatric population
  • Regular assessment of renal function necessary
