Function
- Management of type 2 diabetes mellitus
- Lowers basal and postprandial plasma glucose

Dosage & Administration
- Usual starting dose of 500mg twice a day for adults
- Dosage increases should be made in increments of 500mg or 850mg
- Maximum daily dose of 2000mg for adults
- Swallow extended release tablet whole and never crush, cut or chew
- Special precaution for renal impaired patients
Interaction
- Enhancement of absorption with Nifedipine
- Potential interaction with cationic drugs eliminated by renal tubular secretion
- Effects on Cimetidine pharmacokinetics
- Certain drugs tend to produce hyperglycemia and may lead to loss of glycemic control
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Acute metabolic acidosis
- Severe renal failure
- Acute conditions altering renal function
- Acute or chronic diseases causing tissue hypoxia
- Hepatic insufficiency
- Alcohol intoxication or alcoholism
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders like nausea, vomiting, diarrhea, abdominal pain, loss of appetite
- Liver function test abnormalities or hepatitis
- Skin reactions like erythema, pruritus, urticaria
Pregnancy & Lactation
- Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
- Excretion into human breast milk
- Breastfeeding not recommended during metformin treatment
Precautions & Warnings
- Substantial excretion by the kidney
- Lowering of vitamin B12 level
- Increased risk of hypoglycemia when used in combination with insulin or insulin secretagogue
- Regular monitoring of renal function advised
- Therapy cessation before certain medical procedures
- Annual estimation of Vitamin B-12 levels for continuous therapy patients
- Monitoring for hypoglycemia during combined sulphonylurea therapy
- Caution during cimetidine therapy
- Potential interaction with anticoagulants
Use in Special Populations
- Dosage adjustment based on renal function in elderly subjects
- Confirmation of type 2 diabetes mellitus before treatment initiation in pediatric population
