Indications
- Treatment of type 2 diabetes mellitus

Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Different mechanism of action compared to sulfonylureas
- Does not produce hypoglycemia
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity
- Increases peripheral glucose uptake and utilization
Dosage & Administration
- Individualized dosage based on effectiveness and tolerance
- Maximum recommended daily doses
- Usual starting dose of 500 mg twice a day or 850 mg once a day for adults
- Usual starting dose of 500 mg twice a day for children
- Dosage increases should be made in increments of 500 mg weekly
- Usual starting dose of 500 mg once daily with the evening meal for extended release tablet in adults
- Dose increases in increments of 500 mg weekly for extended release tablet
Interaction
- Absorption enhancement with Nifedipine
- Minimal effects on Nifedipine
- Potential interaction with cationic drugs eliminated by renal tubular secretion
- No effect on Cimetidine pharmacokinetics
- Certain drugs tend to produce hyperglycemia
Contraindications
- Hypersensitivity to the active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Acute conditions affecting renal function
- Conditions causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders such as nausea, vomiting, diarrhea
- Liver function test abnormalities or hepatitis
- Skin reactions
Pregnancy & Lactation
- Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
- Use of insulin recommended during pregnancy
- Excretion into human breast milk
- No adverse effects observed in breastfed newborns/infants
- Breastfeeding not recommended during treatment
Precautions & Warnings
- Excretion by the kidney
- Risk of lactic acidosis increases with renal impairment
- Lower vitamin B12 level
- Increased risk of hypoglycemia when used in combination with insulin
- Regular monitoring of renal function advised
- Discontinuation before surgery and certain clinical investigations
- Not advised in conditions causing dehydration or in patients suffering from serious infections or trauma
Use in Special Populations
- Dosage adjustment in elderly subjects based on renal function
- No effect on growth and puberty detected in controlled clinical studies
- Particular caution recommended for children aged between 10 and 12 years
- Renal function assessment before treatment initiation and regular monitoring
Overdose Effects
- No hypoglycemia seen with doses up to 85 gm
- Lactic acidosis occurred in overdose circumstances
- Hemodialysis as the most effective method to remove lactate and Metcure
