Indications

Pharmacology
- Type of oral antihyperglycemic drug
- Lowers both basal and postprandial plasma glucose
- Mechanism of action different from sulfonylureas
- Does not produce hypoglycemia
- Decreases hepatic glucose production

Precautions & Warnings
- Substantial excretion by the kidney
- Risk of Metformin accumulation and lactic acidosis with renal impairment
- Lowering of vitamin B12 levels
- Increased risk of hypoglycemia when used with insulin or insulin secretagogue
- Regular monitoring of renal function
- Stoppage before surgery and clinical investigations
Use in Special Populations
- Dosage adjustment for elderly subjects based on renal function
- Regular assessment of renal function in all diabetics
- Special caution when prescribing to children aged 10-12 years
- Estimation of creatinine clearance before treatment initiation
- Contradicted in patients with GFR<30 ml/min
Overdose Effects
- No hypoglycemia seen at high doses
