Indications

  • Treatment of type 2 diabetes mellitus in adults
  • Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
  • Reduction of diabetic complications in overweight type 2 diabetic adult patients
  • First-line therapy after diet failure

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers both basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity

Interaction

  • Absence of chronic co-administration information with Furosemide

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Any type of acute metabolic acidosis
  • Severe renal failure
  • Acute conditions with potential to alter renal function
  • Acute or chronic diseases causing tissue hypoxia

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Peripheral neuropathy in patients with vitamin B12 deficiency
  • Taste disturbance
  • Encephalopathy
  • Gastrointestinal disorders
  • Liver function test abnormalities or hepatitis
  • Skin reactions

Pregnancy & Lactation

  • Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
  • Recommendation to use insulin instead of metformin during pregnancy
  • Excretion of metformin into human breast milk
  • No adverse effects observed in breastfed newborns/infants
  • Breastfeeding not recommended during metformin treatment

Precautions & Warnings

  • Substantial excretion by the kidney
  • Lowering of vitamin B12 level
  • Increased risk of hypoglycemia in combination with insulin
  • Advice for regular monitoring of renal function
  • Usage advice for surgery and clinical investigations
  • Not advised in conditions causing dehydration or serious infections
  • Recommendation for annual estimation of Vitamin B-12 levels
  • Monitoring of blood glucose during combined therapy with a sulphonylurea
  • Dose reduction during cimetidine therapy
  • Potential interaction with anticoagulants

Use in Special Populations

  • Adjustment of metformin dosage based on renal function in elderly subjects
  • No effect on growth and puberty detected during controlled clinical studies
  • Caution recommended when prescribing to children aged between 10 and 12 years
  • Necessity of regular assessment of renal function
  • Special caution in situations where renal function may become impaired
  • Contradiction in patients with GFR<30 mL/min

Overdose Effects

  • No hypoglycemia seen with Insimet XR doses up to 85 gm
  • Occurrence of lactic acidosis in such circumstances
  • Hemodialysis as the most effective method for removal of lactate and Insimet XR
