Indications
- Treatment of type 2 diabetes mellitus
- Management of overweight patients with inadequate glycaemic control
- Can be used as monotherapy or in combination with other oral antidiabetic agents or insulin
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
Dosage & Administration
- Dosage must be individualized
- Can be used in adults and children
- Different dosage for immediate release and extended release tablet
- Not to be used in severe renal impairment
Interaction
- May interact with Furosemide and Nifedipine
- Potential interactions with various cationic drugs
- Certain drugs may lead to loss of glycemic control
Contraindications
- Hypersensitivity to the active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Acute conditions affecting renal function
Side Effects
- Blood and lymphatic system disorders: Hemolytic anemia
- Metabolism and nutrition disorders: Lactic acidosis, decrease in vitamin B12 absorption, peripheral neuropathy
- Nervous system disorders: Taste disturbance, encephalopathy
- Gastrointestinal disorders: Nausea, vomiting, diarrhea, abdominal pain
- Hepatobiliary disorders: Isolated reports of liver function test abnormalities
- Skin and subcutaneous tissue disorders: Skin reactions
Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities and perinatal mortality
- Metformin excreted into breast milk
Precautions & Warnings
- Substantially excreted by the kidney
- May lower vitamin B12 levels
- Increases risk of hypoglycemia when used with insulin or insulin secretagogue
- Regular monitoring of renal function advised
- Caution in conditions causing dehydration or serious infections

- Elderly: Dosage adjustment based on renal function
- Pediatric population: Diagnosis confirmation required before treatment

Overdose Effects
- Hypoglycemia not seen at standard doses
