Indications

  • Treatment of type 2 diabetes mellitus
  • Management of type 2 diabetes in overweight patients
  • First-line therapy after diet failure
  • Monotherapy in adults
  • Combination therapy with oral antidiabetic agents or insulin
  • Use in children from 10 years of age and adolescents

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers basal and postprandial plasma glucose
  • Improves insulin sensitivity
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose

Dosage & Administration

  • Individualized dosage based on effectiveness and tolerance
  • Starting dose for adults: 500 mg twice a day or 850 mg once a day
  • Maximum daily dose up to 2000 mg per day
  • Starting dose for children: 500 mg twice a day
  • Usual starting dose of Metformin XR is 500 mg once daily
  • Dose increment and maximum dosage for Metformin XR

Interaction

  • Interaction with Furosemide and Nifedipine
  • Potential interaction with cationic drugs
  • Effects on Cimetidine pharmacokinetics
  • Drugs that may produce hyperglycemia

Contraindications

  • Hypersensitivity to active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Acute conditions altering renal function
  • Conditions causing tissue hypoxia

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Neuropathy in patients with vitamin B12 deficiency
