Medicine Details

Indications

  • Treatment of type 2 diabetes mellitus in adults
  • Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers both basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity by increasing peripheral glucose uptake and utilization

Dosage & Administration

  • Dosage should be individualized based on effectiveness and tolerance
  • Usual starting dose for adults: 500 mg twice a day or 850 mg once a day
  • Maximum dose for adults: 2000 mg per day
  • Usual starting dose for children: 500 mg twice a day
  • Maximum dose for children: 2000 mg per day
  • Metformin extended release tablet should be swallowed whole and not crushed, cut, or chewed

Interaction

  • Enhancement of absorption by Nifedipine
  • Potential interaction with cationic drugs eliminated by renal tubular secretion
  • No effect on Cimetidine pharmacokinetics
  • Certain drugs may produce hyperglycemia and affect glycemic control

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Acute conditions altering renal function
  • Conditions causing tissue hypoxia

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Taste disturbance
  • Encephalopathy
  • Gastrointestinal disorders
  • Liver function test abnormalities or hepatitis
  • Skin reactions

Pregnancy & Lactation

  • Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities and perinatal mortality
  • Metformin excreted into human breast milk
  • Breastfeeding not recommended during metformin treatment

Precautions & Warnings

  • Substantially excreted by the kidney
  • Increased risk of lactic acidosis with impaired renal function
  • Lowering of vitamin B12 levels
  • Increased risk of hypoglycemia in combination with insulin or insulin secretagogue
  • Regular monitoring of renal function advised
  • Consideration for discontinuation before surgery
  • Monitoring of Vitamin B12 levels during continuous therapy
  • Blood glucose monitoring during combined therapy with sulphonylurea
  • Possible interaction with anticoagulants

Use in Special Populations

  • Dosage adjustment based on renal function in elderly subjects
  • No effect on growth and puberty detected in controlled clinical studies
  • Caution recommended when prescribing to children aged between 10 and 12 years
  • Renal function assessment before treatment initiation and regularly thereafter

Overdose Effects

  • Hypoglycemia not seen with doses up to 85 gm
  • Lactic acidosis occurred in such circumstances
  • Hemodialysis as the most effective method for removal
