Indications

  • Treatment of type 2 diabetes mellitus in adults
  • Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
  • Reduction of diabetic complications in overweight type 2 diabetic adult patients

Pharmacology

  • Type 2 diabetes management
  • Lowers basal and postprandial plasma glucose
  • Different mechanism of action from sulfonylureas
  • Does not produce hypoglycemia
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity

Dosage & Administration

  • Dosage must be individualized based on effectiveness and tolerance
  • Starting dose of 500 mg or 850 mg depending on formulation
  • Dosage increases should be made in increments of 500 mg or 850 mg
  • Maximum daily doses based on patient type and formulation
  • Dosage adjustments for renal impaired patients

Interaction

  • Potential interaction with Furosemide and Nifedipine
  • Possible competition with cationic drugs for renal tubular transport systems
  • Minimal effect on Cimetidine pharmacokinetics

Contraindications

  • Hypersensitivity to active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure (GFR <30 mL/min)
  • Acute conditions altering renal function
  • Acute or chronic diseases causing tissue hypoxia

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Peripheral neuropathy in patients with vitamin B12 deficiency
  • Taste disturbance
  • Encephalopathy
  • Gastrointestinal disorders like nausea, vomiting, diarrhea, abdominal pain
  • Liver function test abnormalities
  • Skin reactions like erythema, pruritus, urticaria

Pregnancy & Lactation

  • Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities and perinatal mortality
  • Metformin excreted into human breast milk
  • Breastfeeding not recommended during metformin treatment

Precautions & Warnings

  • Substantial excretion by the kidney
  • Risk of SB-Met accumulation and lactic acidosis with impaired renal function
  • Lowering of vitamin B12 level
  • Increased risk of hypoglycemia when used with insulin
  • Regular monitoring of renal function advised
  • Discontinuation before surgery and clinical investigations
  • Not advised in conditions causing dehydration or in patients with serious infections or trauma

Use in Special Populations

  • Elderly: Adjustment of metformin dosage based on renal function
  • Pediatric population: Diagnosis of type 2 diabetes mellitus should be confirmed before treatment with metformin is initiated
  • Renal function assessment before and during treatment

Overdose Effects

  • No hypoglycemia seen with SB-Met doses up to 85 gm
  • Lactic acidosis occurred in such circumstances
  • Hemodialysis as the most effective method to remove lactate and SB-Met
