Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children and adolescents
- Reduction of diabetic complications in overweight patients
- Monotherapy for type 2 diabetes mellitus
- Combination therapy with other oral antidiabetic agents
- Combination therapy with insulin
Pharmacology
- Lowers basal and postprandial plasma glucose levels
- Different mechanism of action than sulfonylureas
- Does not produce hypoglycemia
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity
Dosage & Administration
- Dosage individualized based on effectiveness and tolerance
- Starting dose for adults: 500 mg twice a day or 850 mg once a day
- Starting dose for children: 500 mg twice a day
- Maximum daily dose for adults: 2550 mg
- Maximum daily dose for children: 2000 mg
- Extended release tablet should be swallowed whole
Interaction
- Enhancement of absorption with nifedipine
- No effect on cimetidine pharmacokinetics
- Potential interaction with cationic drugs
- Certain drugs may produce hyperglycemia
Contraindications
- Hypersensitivity to active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Acute conditions altering renal function
- Conditions causing tissue hypoxia
Side Effects
- Hemolytic anemia (not known)
- Lactic acidosis (very rare)
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Gastrointestinal disorders
- Liver function test abnormalities or hepatitis (very rare)
- Skin reactions
Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy associated with increased risk
- Breastfeeding not recommended during treatment
Precautions & Warnings
- Substantially excreted by the kidney
- May lower vitamin B12 level
- Increased risk of hypoglycemia with insulin combination
- Regular monitoring of renal function advised
- Therapy should be stopped before surgery
- Annual estimation of Vitamin B-12 levels advised
- Blood glucose monitoring during sulphonylurea combination therapy
- Possible interaction with anticoagulants
Use in Special Populations
- Elderly: dosage adjustment based on renal function
- Pediatric population: confirmation of type 2 diabetes mellitus diagnosis before treatment
- Renal function assessment before and during treatment
- Caution in children aged between 10 and 12 years
- Contradicted in patients with GFR < 30 ml/min
Overdose Effects
- Lactic acidosis is a medical emergency
- Hemodialysis for lactate and drug removal
