Indications
- Treatment of type 2 diabetes mellitus
- Management of type 2 diabetes in overweight patients
- Reducing diabetic complications in adults
- Use in children from 10 years of age and adolescents
- Use as monotherapy or in combination with other medication
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
- Does not produce hypoglycemia
Dosage & Administration
- Usual starting dose of 500mg twice a day for adults
- Dosage increments of 500mg/850mg
- Maximum daily dose of 2000mg for adults
- Dosage adjustment for renal impaired patients
- Swallowing extended release tablet whole
Interaction
- Potential interaction with Furosemide and Nifedipine
- Competing for common renal tubular transport systems
- Effect on drugs that can produce hyperglycemia
- Minimal effects on Cimetidine pharmacokinetics
Contraindications
- Hypersensitivity to the active substance
- Acute metabolic acidosis
- Severe renal failure
- Acute conditions altering renal function
- Conditions causing tissue hypoxia

Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy associated with risk of congenital abnormalities
- Excretion of metformin into human breast milk
- Breastfeeding not recommended during metformin treatment
Precautions & Warnings
- Substantially excreted by the kidney
- Potential for decreased renal function in elderly subjects
- Lowering vitamin B12 level
- Increased risk of hypoglycemia when used in combination with insulin
- Regular monitoring of renal function advised
Use in Special Populations
- Dosage adjustment based on renal function in elderly
- Caution in prescribing to prepubescent children
- Contraindicated in patients with GFR < 30 ml/min
Overdose Effects
- Lactic acidosis as a medical emergency
- Hypoglycemia and hemodialysis for treatment
