Indications

  • Treatment of type 2 diabetes mellitus in adults
  • Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
  • Reduction of diabetic complications in overweight type 2 diabetic adult patients

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Improves insulin sensitivity

Dosage & Administration

  • Individualized dosage based on effectiveness and tolerance
  • Usual starting dose for adults: 500 mg twice a day or 850 mg once a day
  • Usual starting dose for children: 500 mg twice a day
  • Usual starting dose for Metformin XR in adults: 500 mg once daily with the evening meal

Interaction

Contraindications

Pregnancy & Lactation

  • Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities and perinatal mortality
  • Metformin excreted into human breast milk
  • No adverse effects observed in breastfed newborns/infants

Precautions & Warnings

  • Substantially excreted by the kidney
  • May lower vitamin B12 level
  • Increases risk of hypoglycemia in combination with insulin

Use in Special Populations

  • Dosage adjustment based on renal function in elderly subjects
  • No effect on growth and puberty during controlled clinical studies

Overdose Effects

  • Hypoglycemia not seen with Verimet doses up to 85 gm
  • Lactic acidosis observed in such circumstances
  • Lactic acidosis is a medical emergency
  • Hemodialysis may be effective for removal of lactate and Verimet
