Medicine Details

Indications

  • Treatment of type 2 diabetes mellitus in overweight patients
  • May be used as monotherapy or in combination with other oral antidiabetic agents or insulin
  • Reduction of diabetic complications in overweight patients

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers both basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Improves insulin sensitivity

Dosage & Administration

  • Must be individualized based on effectiveness and tolerance
  • Usual starting dose for adults - 500mg twice a day or 850mg once a day
  • Usual starting dose for children - 500mg twice a day
  • Maximum daily doses for adults - 2000mg or 2550mg with additional glycemic control
  • Maximum daily doses for children - 2000mg
  • Different dosage for immediate release and extended release tablets
  • Not studied in children for extended release tablets
  • Do not use in patients with eGFR below 30 mL/min/1.73 m^2

Interaction

  • Nifedipine enhances absorption of Verimet
  • Minimal effects on Nifedipine
  • Potential interaction with cationic drugs eliminated by renal tubular secretion

Contraindications

  • Hypersensitivity to the active substance or excipients

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Peripheral neuropathy in patients with vitamin B12 deficiency
  • Taste disturbance
  • Nausea, vomiting, diarrhea, abdominal pain
  • Liver function test abnormalities or hepatitis
  • Skin reactions such as erythema, pruritus, urticaria

Pregnancy & Lactation

  • Uncontrolled diabetes during pregnancy increases risk of congenital abnormalities and perinatal mortality
  • Metformin excreted into human breast milk
  • Breastfeeding not recommended during metformin treatment

Precautions & Warnings

  • Substantially excreted by the kidney
  • Lowers vitamin B12 level
  • Increases risk of hypoglycemia when used with insulin or insulin secretagogue
  • Regular monitoring of renal function advised
  • Therapy should be stopped before surgery and reinstated after control of renal function
  • Caution with dehydration, serious infections, or trauma
  • Annual estimation of Vitamin B-12 levels advised for continuous therapy

Use in Special Populations

  • Dosage adjustment based on renal function in elderly subjects
  • Careful follow-up on effect of metformin on growth and puberty in children
  • Particular caution in children aged 10-12 years
  • Regular assessment of renal function required

Overdose Effects

  • Hypoglycemia not seen with Verimet doses up to 85g
  • Lactic acidosis a medical emergency and must be treated in hospital
  • Hemodialysis most effective method to remove lactate and Verimet
