Description
A potent anti-inflammatory steroid with greater potency than Prednisolone and less tendency to induce sodium and water retention.
Indications
- Treatment of endocrine disorders such as Adrenocortical Insufficiency
- Management of rheumatic disorders like Rheumatoid Arthritis and Ankylosing Spondylitis
- Use in collagen diseases and dermatologic conditions
- Relief of allergy symptoms and ophthalmic diseases
- Management of respiratory diseases and hematological disorders
- Palliative care for neoplastic diseases
- Management of edematous states and gastrointestinal diseases
- Treatment of acute exacerbations of Multiple Sclerosis
Pharmacology
- Pharmacodynamic Properties:
- Potent anti-inflammatory agent
- Inhibits the immune system
- Affects kidney, fluid & electrolyte balance, lipid & protein metabolism, skeletal muscle, cardiovascular system, and more
- Pharmacokinetic Properties:
- High oral bioavailability (82% to 89%)
- Rapid absorption with peak plasma concentration achieved in 1.5 to 2.3 hours
- Wide distribution in tissues including crossing the blood-brain and placental barriers
- Metabolized in the liver to inactive compounds

DosageAndAdministration
- Usual Range: 2-48 mg daily in divided doses
- Initial Dosage: 4-48 mg per day, adjusted based on disease severity
- Maintenance Dosage: Gradual reduction to lowest effective dose
- Treatment of Multiple Sclerosis: 160 mg daily for a week followed by 64 mg every other day for 1 month
- Alternate-day Therapy: Regimen to minimize certain undesirable effects
Interactions
- Erythromycin, Clarithromycin, Phenobarbital, Phenytoin, Rifampin and Ketoconazole inhibit its metabolism
- Estrogens can increase its effect
- Cyclosporin reduces its metabolism while Methipred reduces the metabolism of Cyclosporin
- May impact the effect of blood thinners

Systemic fungal infections and known hypersensitivity to components

- Careful observation of growth and development of infants and children on prolonged therapy
