Description
- Potent anti-inflammatory steroid
- Greater anti-inflammatory potency than Prednisolone
- Less tendency to induce sodium and water retention
- Relative potency to Hydrocortisone at least four to one
Pharmacology
- Potent anti-inflammatory agent
- Profoundly inhibits the immune system
- Binds to and activates intracellular glucocorticoid receptors
- Influences kidney, fluid & electrolyte balance, lipid, protein, and carbohydrate metabolism
- Widely distributed into the tissues
- Crosses the blood-brain barrier and the placental barrier
- Secreted in breast milk
- Metabolized in the liver to inactive metabolites
- Hemodializable
Dosage & Administration
- Usual range is 2-48 mg daily in divided doses
- Initial dosage may vary from 4-48 mg per day depending on the specific disease entity being treated
- Proper maintenance dosage determined by decreasing the initial drug dosage in small decrements
- Multiple Sclerosis treatment: 160 mg daily for a week followed by 64 mg every other day for 1 month
- Alternate-day therapy (ADT) regimen for long-term pharmacologic dose treatment
Interaction
- Inhibited metabolism by Erythromycin, Clarithromycin, Phenobarbital, Phenytoin, Rifampin, and Ketoconazole
- Increased effect with Estrogens, including With control pills
- Reduced metabolism with Cyclosporin
- Potential interaction with blood thinners

- Systemic fungal infections
- Known hypersensitivity to components

Pregnancy & Lactation
- Pregnancy category C
- Use only if potential benefit justifies potential risk to the fetus
- Not adequately evaluated in nursing mothers
Precautions & Warnings
- Possible persistence of adrenocortical insufficiency after discontinuation of therapy
- Enhanced effect on patients with hypothyroidism and in those with cirrhosis
- Cautious use in patients with ocular herpes simplex
- Cautious use in conjunction with aspirin in hypoprothrombinemia
- Careful observation of infants and children on prolonged corticosteroid therapy
Overdose Effects
- Rare reports of acute toxicity and/or death following an overdose
- No specific antidote available
- Supportive and symptomatic treatment
- Monitoring of serum electrolytes
