Indications
- Endocrine Disorders: Primary or Secondary Adrenocortical Insufficiency

Description
Mepcort is a potent anti-inflammatory steroid with greater potency than Prednisolone, and less tendency to induce sodium and water retention.
Pharmacology
- Pharmacodynamic properties: Potent anti-inflammatory agent influencing various systems such as kidney, fluid & electrolyte balance, lipid, protein, and carbohydrate metabolism, immune system, nervous system, and endocrine system
- Pharmacokinetic properties: High bioavailability, rapid absorption, wide tissue distribution, metabolized in the liver, haemodializable
Dosage and administration
- Usual range is 2-48 mg daily in divided doses, depending on the specific disease being treated
- Initial dosage may vary from 4-48 mg per day depending on the specific disease entity being treated
- Maintenance dosage should be determined by decreasing the initial drug dosage in small decrements
- Effective treatment of acute exacerbations of multiple sclerosis with specific dosing regimen
Interaction
- Inhibit metabolism:
- Erythromycin
- Clarithromycin
- Phenobarbital
- Phenytoin
- Rifampin
- Ketoconazole
- Increase effect: Estrogens
- Reduce metabolism: Cyclosporin
- Affect blood thinners: Mepcort may increase or decrease the effect of blood thinners
Contraindications
Systemic fungal infections and known hypersensitivity to components
Side effects
Range from fluid retention, weight gain to growth retardation in children, convulsions, and psychic disturbances
Pregnancy and lactation
Pregnancy category C. Dregs should be given only if the potential benefit justifies the potential risk to the fetus
Precautions and warnings
- Adrenocortical insufficiency: May persist for months after discontinuation of therapy
- Enhanced effect hypothyroidism: Enhanced effect of corticosteroids on patients with hypothyroidism
- Ocular herpes simplex: Corticosteroids should be used cautiously in patients with ocular herpes simplex
- Aspirin use: Aspirin should be used cautiously in conjunction with corticosteroids in hypoprothrombinemia
- Growth and development: The growth and development of infants and children on prolonged corticosteroid therapy should be carefully observed
Overdose effects
Acute toxicity and/or death following an overdose of glucocorticoid are rare. No specific antidote available
