Indications
- Peptic ulcer diseases
- Gastroesophageal reflux diseases
- Ulcer induced by non-steroidal anti-inflammatory drugs (NSAIDs)
- Eradication of Helicobacter pylori (in combination with antibiotics)
- Zollinger-Ellison Syndrome
Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid production
- Binds to H+/K+ATPase enzyme system
- Inhibits basal and stimulated gastric acid secretion
- Effect persists longer than 24 hours
Dosage & Administration
- Oral administration
- 40 mg daily for benign gastric ulcer
- 20-40 mg daily for GERD
- 40 mg daily for duodenal ulcer
- 40 mg twice daily with antibiotics for duodenal ulcer associated with Helicobacter pylori
- 20 mg daily for NSAID-associated gastric or duodenal ulcer
- Initially 80 mg once daily for Zollinger-Ellison Syndrome
- IV injection for duodenal ulcer, gastric ulcer, GERD, prevention of rebleeding in peptic ulcer, prophylaxis of acid aspiration, long-term management of Zollinger-Ellison Syndrome
Interaction
- No significant drug interactions observed in clinical studies
Contraindications
- Known hypersensitivity to any components of the formulation
Side Effects
- Well tolerated in short-term and long-term treatment
- Common side effects: headache, diarrhea
- Rare side effects: abdominal pain, flatulence, rash, insomnia, hyperglycemia
Pregnancy & Lactation
- US FDA Pregnancy Category B
- No adequate and well-controlled studies in pregnant women
- Excreted in human milk, use during pregnancy only if clearly needed
- Decision should be made whether to discontinue nursing or discontinue the drug, considering benefit to the mother
Precautions & Warnings
- Tablet should not be split, chewed, or crushed
- Long-term therapy may lead to malabsorption of cyanocobalamin (Vitamin B12)
- Increased risk of osteoporosis related disorders
Overdose Effects
- No known symptoms of overdosage in humans
- Highly protein bound, not readily dialyzable
- Symptomatic and supportive management recommended
