Description
- Potent anti-inflammatory steroid
- Greater anti-inflammatory potency than Prednisolone
- Less tendency to induce sodium and water retention
- Relative potency to Hydrocortisone is at least four to one
Pharmacology
- Potent anti-inflammatory agent
- Profoundly inhibits the immune system
- Influences kidney and fluid & electrolyte balance
- Affects lipid, protein, and carbohydrate metabolism
- Affects skeletal muscle, cardiovascular system, immune system, nervous system, and endocrine system
- High absolute bioavailability following oral administration
- Rapidly absorbed and achieves maximum plasma concentration in 1.5 to 2.3 hours
- Widely distributed into tissues
- Crosses the blood-brain barrier and placental barrier
- Secreted in breast milk
- Metabolized in the liver to inactive metabolites
- Plasma protein binding in humans is approximately 77%

Dosage & Administration
- Usual range is 2-48 mg daily in divided doses
- Initial dosage may vary from 4-48 mg per day
- Proper maintenance dosage should be determined by decreasing the initial drug dosage in small decrements
- Treatment of acute exacerbations of multiple sclerosis with specific dosage
- Guidelines for treating severe allergy and dermatitis with specific dosage
- Alternate-day therapy (ADT) principles and indications
- Monitoring needed in regard to drug dosage
Interaction
- Inhibition or reduction of metabolism by Erythromycin, Clarithromycin, Phenobarbital, Phenytoin, Rifampin, Ketoconazole, and Cyclosporin
- Increased effect with estrogens
- Effect on blood thinners like Warfarin
Contraindications
- Systemic fungal infections
- Known hypersensitivity to components
Side Effects
- Well-tolerated with few, mild side effects in short courses
- Potential serious side effects with long-term, high doses
- Range from mild annoyances to serious irreversible bodily damage
- Depression of body's adrenal glands with prolonged use
Pregnancy & Lactation
- Pregnancy category C
- Drug evaluation in nursing mothers not adequate
Precautions & Warnings
- Adrenocortical insufficiency may persist after therapy discontinuation
- Enhanced effect on patients with hypothyroidism and in those with cirrhosis
- Cautious use in patients with ocular herpes simplex
- Careful observation of infants' and children's growth and development on prolonged therapy
Overdose Effects
- Rare reports of acute toxicity and/or death following an overdose
- No specific antidote available
- Supportive and symptomatic treatment
