Description
Potent anti-inflammatory steroid with greater potency than Prednisolone and less tendency to induce sodium and water retention
Pharmacology
- Pharmacodynamic properties: Potent anti-inflammatory agent with profound immune system inhibition, affecting various systems like kidney, fluid & electrolyte balance, lipid, protein, and carbohydrate metabolism, skeletal muscle, cardiovascular system, immune system, nervous system, and endocrine system
- Pharmacokinetic properties: High bioavailability, rapid absorption, widely distributed into tissues, crosses barriers, metabolized in the liver, and haemodializable
Dosage & Administration
- Usual range of 2-48 mg daily in divided doses
- Initial dosage of 4-48 mg per day for specific disease entity

Interaction
- Inhibition and reduction of metabolism with erythromycin, clarithromycin, phenobarbital, phenytoin, rifampin, ketoconazole, and cyclosporin
- Increased effect with estrogens including birth control pills
- Effect on blood thinners
Contraindications
Systemic fungal infections and known hypersensitivity to components
Side Effects
- Mild side effects for short courses, potential serious side effects for long-term high doses
- Effects range from fluid retention, weight gain, high blood pressure, to growth retardation in children and psychic disturbances
Pregnancy & Lactation
Category C drug, should be given only if potential benefit justifies the potential risk to the fetus, not adequately evaluated in nursing mothers
Precautions & Warnings
- Adrenocortical insufficiency may persist after discontinuation, mineralocorticoid secretion may be impaired
- Enhanced effect on patients with hypothyroidism and in those with cirrhosis
- Caution in patients with ocular herpes simplex, hypoprothrombinemia, and growth and development observation in infants and children
Overdose Effects
Rare reports of acute toxicity and/or death following an overdose, no specific antidote available, supportive and symptomatic treatment, monitor serum electrolytes
