Pharmacology
Synthetic adrenocortical drug with predominantly glucocorticoid properties. Inhibits Phospholipase A2 enzyme. Absorbed well from the gastrointestinal tract. Metabolized mainly in the liver and excreted in the urine.
Dosage & Administration
- Adult:
- Nephrotic Syndrome: Initial dose of 2 mg/kg/day (maximum 80 mg/day) in divided doses followed by maintenance dose of 0.5 to 1 mg/kg/dose every other day
- Anti-inflammatory: 5 to 60 mg per day in divided doses
- Acute Asthma: 40-60 mg/day PO in single daily dose or divided q12 hr for 3-10 days
- Allergic Conditions: Gradual reduction in dosage over days
- Pediatric:
- Asthma:
- 10 mg to 40 mg orally every 12 hours, based on age
- Anti-inflammatory: 0.05 to 2 mg/kg/day divided 1 to 4 times/day
- Immunosuppression: 0.05 to 2 mg/kg/day divided 1 to 4 times/day

Interaction
Reduced efficacy by various medications. Can cause reduced potassium levels in the blood and interact with digitalis leading to cardiac arrhythmias if hypokalemia occurs. Immunization should be performed carefully.
Contraindications
Systemic infections, hypersensitivity to any ingredient, ocular herpes simplex

- Flushed face or cheeks
- Hiccups
- Increased sweating
Pregnancy & Lactation
Not recommended during pregnancy unless essential. May appear in breast milk and could suppress growth or cause unwanted effects in the infant.
Precautions & Warnings
Special precaution in diabetes, hypertension, psychological disturbances, osteoporosis, post-menopausal women, chronic nephritis. Long-term use can cause various adverse effects.
Overdose Effects
Adverse effects develop after prolonged use. Treatment is symptomatic, and prednisolone dose should be reduced gradually.
