Dosage Form

Dosage Instructions

  • 200 mg tid or 400 mg bid for 7 days (Adults & Children over 10 yrs)
  • 800 mg in the morning and 1-2 gm at night for 2 days (Adults & Children over 10 yrs)
  • 2 gm as a single dose for 1 days (Adults & Children over 10 yrs)
  • 100 mg tid for Children 7-10 yrs
  • 100 mg bid for Children 3-7 yrs
  • 50 mg tid for Children 1-3 yrs
  • 800 mg tid for 5 days (Adults & Children over 10 yrs)
  • 400 mg tid for Children 7-10 yrs
  • 200 mg qid for Children 3-7 yrs
  • 200 mg tid for Children 1-3 yrs
  • 400-800 mg tid for 5-10 days (Adults & Children over 10 yrs)
  • 200-400 mg tid for Children 7-10 yrs

Mode of Action

Metronidazole, the active ingredient, is an imidazole class antibacterial drug that is classified therapeutically as an antiprotozoal agent. It interacts with DNA and exhibits bactericidal actions.

Indications

  • Prevention of post-operative infections due to anaerobic bacteria
  • Treatment of septicaemia, bacteraemia, peritonitis, brain abscess, pelvic abscess, pelvic cellulitis, post-operative wound infections caused by anaerobes
  • Urogenital trichomoniasis
  • Bacterial vaginosis
  • Amoebiasis (intestinal, extra-intestinal, and in symptomless cyst passers)
  • Giardiasis
  • Acute ulcerative gingivitis
  • Anaerobically infected leg ulcers and pressure sores
  • Acute dental infections due to anaerobic organisms
  • Antibiotic-associated pseudomembranous colitis

Interaction

  • Disulfiram: Psychotic reactions reported when used concurrently
  • Alcohol: Consumption during therapy and for at least one day afterwards should be avoided
  • Oral anticoagulant therapy (warfarin type): Potentiation of the anticoagulant effect, increased hemorrhagic risk
  • Lithium: Metronidazole may increase plasma levels of lithium
  • Cyclosporin: Close monitoring of serum levels and serum creatinine is necessary
  • Phenytoin or phenobarbital: Increased elimination of Metronidazole resulting in reduced plasma levels
  • 5-Fluorouracil: Reduced clearance leading to increased toxicity
  • Busulfan: Metronidazole may increase plasma levels of busulfan, leading to severe toxicity

Contraindications

Metronidazole is contraindicated in patients with a history of hypersensitivity to Metronidazole or other Nitroimidazole derivatives.

US FDA Pregnancy Category of Metronidazole is B. Caution is advised during pregnancy and nursing.

Precautions & Warnings

  • Regular hematological tests recommended if administered longer than usual duration
  • Caution advised for patients with hepatic encephalopathy
  • Patients should be warned that Metrogard may darken urine

Special Populations

  • Hepatic Impairment: Dosage adjustment and caution advised for patients with advanced hepatic insufficiency
  • Renal Impairment: No routine adjustment in the dosage needed for patients with renal failure undergoing intermittent peritoneal dialysis (IDP) or continuous ambulatory peritoneal dialysis (CAPD)

Overdose Effects

Symptomatic and supportive treatment should be instituted in case of suspected massive overdoses. No specific antidote available.
