Indications

Pharmacology

Pantoprazole is a proton pump inhibitor that suppresses the final step in gastric acid production by covalently binding to the H+/K+-ATPase enzyme system at the surface of the gastric parietal cell. This effect leads to inhibition of both basal and stimulated gastric acid secretion, irrespective of the stimulus that persists longer than 24 hours.

Dosage & Administration

  • 40 mg daily in the morning for 4 weeks for benign gastric ulcer
  • 20-40 mg daily in the morning for 4 weeks for gastro-esophageal reflux disease
  • 40 mg daily in the morning for 2 weeks for duodenal ulcer
  • 40 mg twice daily in association with antimicrobial agents for duodenal ulcer associated with Helicobacter pylori
  • 20 mg daily for long-term NSAID treatment
  • 80 mg once daily for Zollinger-Ellison Syndrome
  • 40 mg once daily for 7-10 days for duodenal ulcer and gastric ulcer IV injection
  • 40 mg once daily for 7-10 days for gastroesophageal reflux disease IV injection
  • IV 80 mg followed by infusion for 72 hours for prevention of rebleeding in peptic ulcer
  • 80 mg IV every 12 h for 24 h for prophylaxis of acid aspiration
  • 80 mg IV every 12 hours for long-term management of Zollinger-Ellison Syndrome and other pathological hypersecretory conditions

No significant drug interactions observed in clinical studies

Contraindications

Pantoprazole is contraindicated in patients with known hypersensitivity to any of the components of the formulation

Precautions & Warnings

  • Tablet should not be split, chewed or crushed
  • Long-term therapy may lead to malabsorption of Vitamin B12
  • May increase the risk of osteoporosis related disorders

Overdose Effects

No known symptoms of overdosage in humans. Highly protein bound and not readily dialyzable.

Reconstitution

  • IV injection should be given as a slow intravenous injection
  • IV infusion should be given as an intravenous infusion over a period of approximately 15 minutes
  • Reconstituted solution may be stored at room temperature for a maximum of 4 hours
