Description
Oral and topical antifungal treatment for candidiasis, dermatomycoses, and gram-positive bacterial superinfections
Indications
- Treatment of oral and gastrointestinal candidiasis
- Eradication of fungal colonization in the mouth and gastrointestinal tract
- Treatment of superinfections due to gram-positive bacteria
- Treatment of tinea pedis (athlete's foot), tinea corporis, tinea manuum, tinea cruris, candidial infections, tinea versicolor, skin infections, perianal infections, stomatitis angularis, balanoposthitis, otitis externa
Pharmacology
- Pharmacodynamics: Antifungal and antibacterial activity against common dermatophytes and yeasts, inhibition of ergosterol biosynthesis, disruption of fungal cell membrane
- Pharmacokinetics:
- Oral Bioavailability: Low (25-30%)
- Plasma Protein Binding: 88.2%
- Metabolism: Largely metabolized
- Elimination Half-life: 20-25 hours
Dosage
- Miconazole Oral Gel:
- Infants 4-24 months: 1.25 ml 4 times a day after meals
- Adults and children 2 years and older: 2.5 ml 4 times a day after meals
- Gastrointestinal tract candidosis for infants and adults: 20 mg per kg body weight per day in four divided doses, not to exceed 250 mg 4 times daily
- Miconazole Cream: Applied twice daily for tinea pedis, tinea cruris, tinea corporis, and cutaneous candidiasis; once daily for tinea versicolor
Administration
Direct application to affected area, keeping in contact for at least a week after symptoms disappear
Interaction
- Avoid concomitant use with Terfenadine, Astemizole, and Cisapride
- May delay metabolism of Phenytoin and Cyclosporine
Contraindications
Known hypersensitivity to miconazole

Precautions & Warnings
Monitor anticoagulant and phenytoin levels, ensure gel does not obstruct the throat in infants and young children

Accidental overdosage may cause vomiting and diarrhea, treatment is symptomatic and supportive
