Therapeutic Class

Dosage & Administration

  • 40 mg daily for benign gastric ulcer for 4 weeks
  • 20-40 mg daily for gastro-esophageal reflux disease for 4 weeks
  • 40 mg daily for duodenal ulcer for 2 weeks
  • 40 mg twice daily with antimicrobial agents for duodenal ulcer associated with Helicobacter pylori
  • 20 mg daily for prophylaxis of NSAID-associated gastric or duodenal ulcer
  • 80 mg once daily for Zollinger-Ellison Syndrome

IV Injection Dosage

  • 40 mg once daily for duodenal ulcer and gastric ulcer for 7-10 days
  • 40 mg once daily for gastroesophageal reflux disease associated with a history of erosive esophagitis for 7-10 days
  • IV 80 mg for prevention of rebleeding in peptic ulcer, followed by 8 mg/hour infusion for 72 hours
  • 80 mg for prophylaxis of acid aspiration
  • 80 mg IV every 12 hours for long-term management of Zollinger-Ellison Syndrome

Indications

  • Peptic ulcer diseases
  • Gastroesophageal reflux diseases
  • Ulcer induced by non-steroidal anti-inflammatory drugs (NSAIDs)
  • Eradication of Helicobacter pylori (in combination with antibiotics)
  • Zollinger-Ellison Syndrome

Pharmacology

  • Pantoprazole is a proton pump inhibitor that suppresses the final step in gastric acid production by binding to the H+/K+ATPase enzyme system at the surface of the gastric parietal cell.

Interaction

  • No significant drug interactions observed in clinical studies

Contraindications

  • Patients with known hypersensitivity to any of the components of the formulation

Side Effects

  • Well tolerated in both short-term and long-term treatment
  • Common side effects include headache and diarrhea
  • Rare side effects include abdominal pain, flatulence, rash, insomnia, and hyperglycemia

Pregnancy & Lactation

  • Pantoprazole is US FDA Pregnancy Category B
  • No adequate and well-controlled studies in pregnant women
  • Excreted in human milk

Precautions & Warnings

  • Tablet should not be split, chewed or crushed
  • Long-term therapy may lead to malabsorption of cyanocobalamin (Vitamin B12) and increase the risk of osteoporosis related disorders

Overdose Effects

  • No known symptoms of overdosage in humans
  • Trupan is highly protein bound and not readily dialyzable
  • No specific therapy recommended, apart from symptomatic and supportive management

Reconstitution

  • IV injection should be given over a period of at least 2 to 5 minutes
  • Freshly prepared solution should be used within 4 hours
  • IV infusion should be given over a period of approximately 15 minutes
  • Reconstituted solution may be stored at room temperature for a maximum of 4 hours prior to further dilution
  • Admixed solution may be stored at room temperature and must be used within 24 hours from initial reconstitution
