Indications
- Treatment of major depressive disorder (MDD)
Pharmacology
- Mechanism of action is unknown
- Enhances central noradrenergic and serotonergic activity
- Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
- Antagonist of 5-HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
- Antagonist of histamine (H<sub>1</sub>) receptors
- Moderate peripheral α1-adrenergic antagonist
- Moderate antagonist at muscarinic receptors
- Rapid and well-absorbed after oral administration
- Mean half-life of elimination is 20-40 hours
- Extensively metabolized and eliminated via the urine and faeces
Dosage & Administration
- Recommended starting dose is 15 mg/day
- Administered in a single dose, preferably in the evening or prior to sleep
- Effective dose range is generally 15 to 45 mg/day
- Dose changes should be made at intervals of less than 1 to 2 weeks
- Not recommended for use in children

Interaction
- Clinically significant drug-drug interactions with Monoamine Oxidase Inhibitors (MAOI) & other serotonergic drugs
- May interrupt the metabolism or activity of Carbamazepine, Phenytoin or Cimetidine
- Patients should avoid Alcohol & Diazepam
Contraindications
- Contraindicated in patients with a known hypersensitivity to Mirtazapine or to any of the excipients
- Concomitant use with a monoamine oxidase (MAO) inhibitor is contraindicated

Pregnancy & Lactation
- Pregnancy Category-C
- Patients should notify their physician if they become pregnant or intend to become pregnant during therapy
- Patients should notify their physician if they are breastfeeding an infant
Precautions & Warnings
- Patients, families, and caregivers should be alert to the emergence of various symptoms
- Patients should be warned about the risk of developing agranulocytosis
- May impair judgment, thinking, and motor skills
- Clinically significant ALT (SGPT) elevations may occur
