Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhances central noradrenergic and serotonergic activity
  • Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Potent antagonist of 5- HT2 and 5-HT3 receptors
  • Moderate peripheral α1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Potent antagonist of histamine (H1) receptors
  • Extensively metabolized and eliminated via urine and faeces

Dosage & Administration

  • Recommended starting dose of 15 mg/day
  • Administration in a single dose, preferably in the evening or prior to sleep
  • Effective dose range of 15 to 45 mg/day
  • Dose changes should be made at intervals of less than 1 to 2 weeks
  • Do not recommended for use in children

Interaction

  • Clinically significant drug-drug interactions with MAOIs & other serotonergic drugs such as tryptophan, triptans, linezolid, serotonin reuptake inhibitors, venlafaxine, lithium, tramadol, or St. John's wort
  • May interrupt the metabolism or activity of Carbamazepine, Phenytoin or Cimetidine
  • Patient should avoid Alcohol & Diazepam

Contraindications

  • Contraindicated in patients with known hypersensitivity to Mirtazapine or to any of the excipients
  • Concomitant use with a monoamine oxidase (MAO) inhibitor is contraindicated

Pregnancy & Lactation

  • Pregnancy Category-C
  • Patients should notify their physician if they become pregnant or intend to become pregnant during Mirtazapine therapy
  • Patients should notify their physician if they are breastfeeding an infant

Precautions & Warnings

  • Patients, their families, and their caregivers should be encouraged to be alert to the emergence of certain symptoms
  • Warned about the risk of developing agranulocytosis
  • May impair judgment, thinking, and motor skills due to sedative effect
  • Clinically significant ALT (SGPT) elevations (≥3 times the upper limit of the normal range) may occur
