Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhances central noradrenergic and serotonergic activity
  • Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Potent antagonist of 5-HT2 and 5-HT3 receptors
  • Potent antagonist of histamine (H1) receptors
  • Moderate peripheral α1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Rapid and well-absorbed after oral administration
  • Mean half-life of elimination is 20-40 hours
  • Displays linear pharmacokinetics within the recommended dose range
  • Extensively metabolized and eliminated via urine and feces

Dosage & Administration

  • Recommended starting dose is 15 mg/day
  • Effective dose range is generally 15 to 45 mg/day
  • The half-life of elimination justifies once-a-day dosing
  • Dose changes should be made at intervals of less than 1 to 2 weeks
  • Not recommended for use in children

Interaction

  • Clinically significant drug-drug interactions with MAO inhibitors and other serotonergic drugs
  • May interrupt the metabolism or activity of certain medications
  • Should avoid Alcohol and Diazepam while taking Mitaprex

Contraindications

  • Contraindicated in patients with known hypersensitivity to Mirtazapine or any of the excipients
  • Concomitant use with MAO inhibitors is contraindicated
  • Should not be used within 14 days of initiating or discontinuing therapy with an MAOI

Side Effects

  • Common side effects include dizziness, drowsiness, dry mouth, increased appetite, and weight gain

Pregnancy & Lactation

  • Pregnancy Category-C
  • Patients should notify their physician if they become pregnant or intend to become pregnant during Mirtazapine therapy
  • Patients should notify their physician if they are breastfeeding an infant

Precautions & Warnings

  • Patients should be alert to the emergence of certain behavioral changes and worsening of depression
  • Warned about the risk of developing agranulocytosis
  • May impair judgment, thinking, and motor skills due to sedative effect
  • Clinically significant ALT (SGPT) elevations may occur
