Description
- Suppresses gastric acid secretion
- Treatment of various gastrointestinal conditions
Indications
- Active duodenal ulcer
- Active benign gastric ulcer
- Symptomatic erosive or ulcerative gastro-esophageal reflux disease (GERD)
- Gastro-esophageal Reflux Disease Long-term Management (GERD Maintenance)
- Symptomatic treatment of moderate to very severe gastro-esophageal reflux disease (symptomatic GERD)
- Zollinger-Ellison Syndrome
- Eradication of Helicobacter pylori in patients with peptic ulcer disease
Pharmacology
- Inhibits the gastric H+/K+-ATPase
- Characterized as a gastric proton-pump inhibitor
Dosage
- Oral dose for active duodenal ulcer and active benign gastric ulcer is 20 mg once daily
- Oral dose for erosive or ulcerative GERD is 20 mg once daily for 4-8 weeks
- Long-term management: 20 mg or 10 mg once daily
- Treatment of GERD in pediatric patients: 5 mg or 10 mg once daily
- Starting dose for Zollinger-Ellison Syndrome is 60 mg once daily
- Eradication of H. pylori: 20 mg twice daily in combination with other medications
Administration
- Tablets should be taken in the morning before eating
- Should be swallowed whole
Interaction
- Potential interaction with ketoconazole or itraconazole
- No interaction with liquid antacids
- Not recommended to be co-administered with atazanavir
Contraindications
- Hypersensitivity to the active substance or excipients
- Contra-indicated in pregnancy and breastfeeding
Side Effects
- Well-tolerated in short-term and long-term studies
- Possible side effects include headache, diarrhea, abdominal pain, vomiting, constipation, dry mouth, changes in appetite, muscle pain, drowsiness, and dizziness
Pregnancy & Lactation
- US FDA pregnancy category 'C'
- No adequate and well-controlled studies in pregnant women
- Likely to be excreted in human milk
Precautions & Warnings
- Patients on long-term treatment should be kept under regular surveillance
- May modestly increase the risk of fractures
- Risk of cross-hypersensitivity reactions
- Reports of blood dyscrasias and hepatic enzyme abnormalities
- Caution in patients with severe hepatic dysfunction
- Treatment may possibly increase the risk of gastrointestinal infections
- Severe hypomagnesaemia reported after prolonged use
- May reduce the absorption of vitamin B12
- May be associated with subacute cutaneous lupus erythematosus (SCLE)
- Increased Chromogranin A (CgA) level may interfere with investigations for neuroendocrine tumours
Use in Special Populations
- No dosage adjustment for patients with renal or hepatic impairment
- Not recommended for use in children
Overdose Effects
- Effects are generally minimal and reversible without further medical intervention
- No specific antidote
- Extensively protein bound and not dialysable
