Indications

Pharmacology

  • Selective and orally active leukotriene receptor antagonist
  • Inhibition of cysteinyl leukotriene receptor (CysLT1)
  • Correlation with the pathophysiology of asthma & allergic rhinitis
  • Contributing to signs and symptoms of asthma such as airway edema and altered cellular activity

Dosage & Administration

  • 10 mg tablet once daily for adults and adolescents 15 years of age and older
  • 5 mg tablet once daily for pediatric patients 6 to 14 years of age
  • 4 mg tablet once daily for pediatric patients 2 to 5 years of age
  • 4 mg oral granules once daily for pediatric patients 6 months to 5 years of age

Use in Pediatric Patients

  • Established safety and efficacy in pediatric patients with asthma 6 months to 14 years of age
  • Similar safety and efficacy profiles to those in adults

Hepatic Insufficiency

  • No dosage adjustment required for patients with mild-to-moderate hepatic insufficiency

Renal Insufficiency

  • No dosage adjustment recommended for patients with renal insufficiency

Elderly Use

  • Similar pharmacokinetic profile and oral bioavailability to younger adults
  • Slightly longer plasma half-life in the elderly
  • No dosage adjustment required for the elderly

Interaction

  • No apparent increase in adverse reactions when administered with other asthma therapies
  • No clinically important effects on the pharmacokinetics of specified drugs, including theophylline, prednisone, and oral contraceptives
  • Used concomitantly with a wide range of commonly prescribed drugs without evidence of clinical adverse interactions

Contraindications

  • Hypersensitivity to any component of this product

Side Effects

  • Common: Diarrhoea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
  • Uncommon: Akathisia, anxiety, arthralgia, asthenia, depression, dizziness, dry mouth, malaise, muscle complaints, oedema, seizure, sleep disorders
  • Rare: Angioedema, concentration impaired, hallucination, memory loss, palpitations, suicidal tendencies, tremor

Pregnancy & Lactation

  • Crosses the placenta following oral dosing in rats and rabbits
  • Caution advised when given to a nursing mother

Precautions & Warnings

  • Not indicated for use in the reversal of bronchospasm in acute asthma attacks
  • Appropriate rescue medication should be available
  • Therapy can be continued during acute exacerbations of asthma
  • Not to be abruptly substituted for inhaled or oral corticosteroids
  • Not to be used as monotherapy for the treatment and management of exercise induced bronchospasm
  • Caution in patients with known aspirin sensitivity

Overdose Effects

  • No adverse experiences in the majority of overdosage reports
  • Adverse experiences consistent with the safety profile of Amikast
  • Usual supportive measures recommended in the event of overdose
