Indications

Pharmacology

  • Montelukast is a selective and orally active leukotriene receptor antagonist
  • Inhibition of the cysteinyl leukotriene receptor (CysLT1)
  • Inhibition of cysteinyl leukotrienes (LTC4, LTD4, LTE4) released from mast cells and eosinophils
  • Correlation with the pathophysiology of asthma & allergic rhinitis

Dosage & Administration

  • Adult dosage: 10 mg tablet once daily
  • Pediatric dosage: 5 mg tablet for 6-14 years, 4 mg tablet for 2-5 years, 4 mg oral granules for 6 months-5 years
  • No dosage adjustment in mild-to-moderate hepatic insufficiency or renal insufficiency
  • Similar pharmacokinetic profile in elderly

Interaction

  • No clinically important effects on the pharmacokinetics of theophylline, prednisone, oral contraceptives, terfenadine, digoxin, and warfarin
  • Used concomitantly with thyroid hormones, sedative hypnotics, NSAIDs, benzodiazepines, and decongestants
  • Phenobarbital decreased AUC of Montelukast by approximately 40%

Contraindications

  • Hypersensitivity to any component of this product

Side Effects

  • Common: Diarrhoea, fever, headache, nausea, skin reactions
  • Uncommon: Anxiety, depression, dizziness, dry mouth, irritability, muscle complaints
  • Rare: Angioedema, disorientation, hallucination, hepatic disorders, memory loss, pulmonary eosinophilia, suicidal tendencies

Pregnancy & Lactation

  • Crosses the placenta in rats and rabbits
  • Use during pregnancy only if clearly needed
  • Caution when given to a nursing mother

Precautions & Warnings

  • Not for use in the reversal of bronchospasm in acute asthma attacks
  • Appropriate rescue medication should be available
  • Therapy can be continued during acute exacerbations of asthma
  • Should not be abruptly substituted for inhaled or oral corticosteroids
  • Not for monotherapy in the treatment of exercise-induced bronchospasm
  • Use with caution in aspirin-sensitive patients

Overdose Effects

  • No adverse experiences in the majority of overdose reports
  • Usual supportive measures recommended in case of overdose
