Description

  • Pulmont is indicated for prophylaxis and chronic treatment of asthma, acute prevention of Exercise-Induced Bronchoconstriction (EIB), relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis
  • Montelukast is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene receptor, which is correlated with the pathophysiology of asthma & allergic rhinitis
  • Dosage for adults and adolescents 15 years of age and older: Montelukast 10 mg tablet once daily, for pediatric patients 6 to 14 years of age: Montelukast 5 mg tablet once daily, for pediatric patients 2 years to 5 years of age: Montelukast 4 mg tablet once daily, for pediatric patients 6 months to 5 years of age: Montelukast 4 mg oral granules once daily
  • Pulmont has been administered with other therapies routinely used in the prophylaxis and chronic treatment of asthma with no apparent increase in adverse reactions; in drug interaction studies, it did not have clinically important effects on the pharmacokinetics of certain drugs
  • Common side effects include diarrhea, fever, gastrointestinal discomfort, headache, nausea, vomiting, and upper respiratory tract infection
  • Montelukast crosses the placenta following oral dosing in rats and rabbits, caution should be exercised when given to a nursing mother
  • Pulmont is not indicated for use in the reversal of bronchospasm in acute asthma attacks and should not be abruptly substituted for inhaled or oral corticosteroids
  • No adverse experiences in the majority of overdosage reports; abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity were the most frequently occurring adverse experiences

Pharmacology

  • Selective and orally active leukotriene receptor antagonist
  • Inhibits the cysteinyl leukotriene receptor (CysLT1)
  • Correlated with the pathophysiology of asthma & allergic rhinitis
  • Inhibits cysteinyl leukotrienes and leukotriene receptor occupation

Dosage and administration

  • Adults and adolescents:
    • Montelukast 10 mg tablet once daily
  • Pediatric patients:
    • 6 to 14 years of age: Montelukast 5 mg tablet once daily
    • 2 years to 5 years of age: Montelukast 4 mg tablet once daily
    • 6 months to 5 years of age: Montelukast 4 mg oral granules once daily (directly in the mouth or mixed with cold water or soft food at room temperature)
  • Hepatic insufficiency: No dosage adjustment required in patients with mild-to-moderate hepatic insufficiency
  • Renal insufficiency: No dosage adjustment recommended in patients with renal insufficiency
  • Elderly use:
    • The pharmacokinetic profile and oral bioavailability of a single 10-mg oral dose of montelukast are similar in elderly and younger adults
    • No dosage adjustment required

Interaction

  • Administered with other therapies routinely used in the prophylaxis and chronic treatment of asthma with no increase in adverse reactions
  • No clinically important effects on the pharmacokinetics of theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, and warfarin
  • Used concomitantly with a wide range of commonly prescribed drugs in clinical studies without clinical adverse interactions

Precautions and warnings

  • Not indicated for use in the reversal of bronchospasm in acute asthma attacks
  • Appropriate rescue medication should be available
  • Therapy can be continued during acute exacerbations of asthma
  • Pulmont should not be abruptly substituted for inhaled or oral corticosteroids
  • Not to be used as monotherapy for the treatment and management of exercise-induced bronchospasm
  • Patients with known aspirin sensitivity should continue avoidance of aspirin or non-steroidal anti-inflammatory agents

Overdose effects

  • No adverse experiences in the majority of overdosage reports
  • Adverse experiences may include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity
  • Usual supportive measures should be employed in the event of overdose
