Indications
- Treatment of active duodenal ulcer
- Treatment of active benign gastric ulcer

Pharmacology
- Suppresses gastric acid secretion by inhibiting the gastric H+/K+-ATPase at the secretory surface of the gastric parietal cell
- Characterized as a gastric proton-pump inhibitor
- Targets the acid (proton) pump within the parietal cell
Dosage
- Recommended oral dose for active duodenal ulcer and active benign gastric ulcer is 20 mg once daily in the morning
- Recommended oral dose for erosive or ulcerative gastro-esophageal reflux disease (GERD) is 20 mg once daily for four to eight weeks
- For long-term management, a maintenance dose of rabeprazole sodium 20 mg or 10 mg once daily can be used
- Symptomatic treatment of moderate to very severe gastro-esophageal reflux disease: 10 mg once daily, with the option for on-demand regimen when needed
- Treatment of Zollinger-Ellison Syndrome: Recommended adult starting dose is 60 mg once a day
Administration
- Rabeprazole tablets should be taken in the morning, before eating
- Tablets should not be chewed or crushed, but should be swallowed whole
Interaction
- Potential interaction with compounds whose absorption is pH dependent
- Potential interaction with ketoconazole or itraconazole resulting in decreased antifungal plasma levels
- Absorption of atazanavir is pH-dependent, therefore co-administration with proton pump inhibitors should be avoided
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Contraindicated in pregnancy and during breastfeeding
Side Effects
- Well-tolerated in both short-term and long-term studies
- Potential side effects include headache, diarrhea, abdominal pain, vomiting, constipation, dry mouth, increased or decreased appetite, muscle pain, drowsiness, and dizziness
Pregnancy & Lactation
- US FDA pregnancy category 'C'
- No adequate and well-controlled studies in pregnant women
- Likely excreted in human milk, decision on use during breastfeeding should be carefully considered
Precautions & Warnings
- Symptomatic response to therapy does not preclude the presence of gastric or esophageal malignancy
- Patients on long-term treatment should be kept under regular surveillance
- May modestly increase the risk of hip, wrist, and spine fractures
- A risk of cross-hypersensitivity reactions with other proton pump inhibitors or substituted benzimidazoles cannot be excluded
- Risk of blood dyscrasias and hepatic enzyme abnormalities
- Caution advised for patients with mild to moderate hepatic impairment
- Possible increase in the risk of gastrointestinal infections
- Severe hypomagnesemia has been reported in patients treated with PPIs for at least three months
Use in Special Populations
- No dosage adjustment is necessary for patients with renal or hepatic impairment
- Not recommended for use in children due to lack of data on safety and efficacy
Overdose Effects
- Effects are generally minimal, reversible, and representative of the known adverse event profile
- No specific antidote is known, treatment should be symptomatic and supportive
