Indications

Pharmacology
- Mechanism of action involves blocking COX-1 and COX-2 enzymes
- Decreased prostaglandin synthesis
- COX-2 enzyme mediates antipyretic, analgesic, and anti-inflammatory properties
- COX-1 enzyme is linked to gastrointestinal upset and renal toxicities
Dosage & Administration
- Oral administration with water after meals
- Adult dosage for different conditions
- Children dosage for juvenile rheumatoid arthritis
- Application of gel 2-6 times a day as required
- Not recommended for use in children
- Not recommended for children under 2 years of age
Interaction
- Diminishes antihypertensive effect of ACE inhibitors
- Delays the absorption of Orxen with antacids & sucralfate
- Increases adverse effects when used with aspirin
- Reduces the natriuretic effect of diuretics
- Enhances the toxicity of methotrexate
- Increases the risk of GI bleeding with warfarin and selective serotonin reuptake inhibitors (SSRI)
Contraindications
- Contraindicated in patients with known hypersensitivity to naproxen
- Not to be given to patients who have experienced asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Contraindicated for the treatment of perioperative pain in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Gastrointestinal: Heartburn, abdominal pain, nausea, diarrhea, dyspepsia
- Central Nervous System: Headache, vertigo, drowsiness
- Dermatological: Pruritus (itching), purpura
- Cardiovascular: Edema, palpitation
- Others: Visual disturbances, hearing disturbances
Pregnancy & Lactation
- Adverse effect on the fetus through pharmacological properties
- Causes a delay in parturition in animals
- Associated with premature closure of the ductus arteriosus and severe pulmonary hypertension in infants born to mothers taking naproxen
- Excreted in small amounts in breast milk for nursing mothers
- Amount distributed into breast milk is considered by some authorities to be too small to be harmful to a breast-fed infant
Precautions & Warnings
- Long-term administration may result in renal papillary necrosis and other abnormal renal pathology
- Reports of acute interstitial nephritis, hematuria, proteinuria, and occasionally nephrotic syndrome
- Eliminated primarily by the kidneys
- Should be used with caution in patients with significantly impaired renal function
- Monitoring of serum creatinine and/or creatinine clearance is advised in these patients
- Reports of moderate to severe jaundice attributed to Orxen
