Description
- Relieves signs & symptoms of Osteoarthritis, Rheumatoid Arthritis, Ankylosing spondylitis
- Decreases risk of developing gastric ulcers
- Consists of an immediate release Esomeprazole Magnesium layer & an enteric-coated Naproxen core
Dosage
- Adult dosage: One tablet twice daily of either: 375 mg naproxen/20 mg of esomeprazole; or 500 mg naproxen/20 mg of esomeprazole
- Adolescent dosage (weight-dependent)
Administration
- Tablet should be taken at least 30 minutes before meals
Interaction
- Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
- Concomitant use with warfarin may increase risk of bleeding
- May interfere with absorption of drugs like Ketoconazole, iron salts and digoxin
- Administration with high-fat food may affect absorption
Contraindications
- Known hypersensitivity to any tablet component
- History of allergic-type reactions after taking aspirin or other NSAIDs
- Use during peri-operative period in the setting of CABG surgery
Side Effects
- Decreases occurrence of gastric ulcers and GI adverse events compared to Naproxen alone
- May be associated with small increased risk of arterial thrombotic events
- May cause GI disturbances, oedema, hypertension, cardiac failure
- Less frequently observed adverse events include gastritis, gastrointestinal bleeding, diarrhoea, constipation
Pregnancy & Lactation
- Should not be used during the third trimester of pregnancy
- Should not be used during breastfeeding
- Use may impair female fertility
- Use during first and second trimester only if potential benefit outweighs potential risk to fetus
Precautions & Warnings
- Avoid combination with other NSAIDs
- Minimize dose and duration
- Use with caution in older people and patients with certain conditions
- Monitor patients for adverse reactions especially in older people and those with gastrointestinal disease
- Appropriate monitoring required for patients with history of hypertension, heart failure, and renal impairment
Use in Special Populations
- Use caution and consider dosage adjustment in elderly patients
- Not recommended for patients with moderate to severe or severe renal impairment
- Monitor patients with mild to moderate hepatic impairment
- Not recommended in patients with severe hepatic impairment
Overdose Effects
- Symptoms may include lethargy, dizziness, abdominal discomfort, nausea, renal dysfunction
- No specific antidotes
- Patients should receive symptomatic and supportive care
- Forced diuresis, alkalinization of urine or hemoperfusion may not be useful due to high protein binding
