Indications

  • Osteoarthritis
  • Rheumatoid arthritis
  • Ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Proton pump inhibitor
  • Inhibits gastric acid secretion
  • Suppresses gastric acid secretion
  • Inhibits the prostaglandin synthesis

Dosage

  • Adult dosage: One tablet twice daily
  • Juvenile Idiopathic Arthritis dosage based on weight

Administration

  • Take at least 30 minutes before meals
  • Do not split, chew, crush or dissolve the tablet

Interaction

  • Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • Concomitant use with warfarin may increase the risk of bleeding
  • Inhibition of gastric acid secretion may interfere with the absorption of certain drugs
  • Administration with high-fat food affects absorption

Contraindications

  • Known hypersensitivity to any component of the tablet
  • History of allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft surgery

Side Effects

  • Gastrointestinal side effects
  • Decreased incidence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
  • Increased risk of arterial thrombotic events
  • Reported adverse events include oedema, hypertension, and cardiac failure

Pregnancy & Lactation

  • Not recommended during the third trimester of pregnancy
  • Should not be used during breastfeeding
  • May impair female fertility

Precautions & Warnings

  • Avoid use with other NSAIDs
  • Minimize undesirable effects by using the lowest effective dose for the shortest duration
  • Use with caution in older people and patients with specific conditions

Use in Special Populations

  • Elderly patients may require dosage adjustment
  • Not recommended for use in patients with moderate to severe renal impairment
  • Monitor patients with hepatic insufficiency closely

Overdose Effects

  • Symptoms may include lethargy, dizziness, epigastric pain, and gastrointestinal bleeding
  • Patients should be managed by symptomatic and supportive care
  • No specific antidote is known
