Indications

  • Osteoarthritis
  • Rheumatoid arthritis
  • Ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers.

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Prostaglandin synthesis inhibition
  • Proton pump inhibitor
  • Suppression of gastric acid secretion

Dosage

  • Use lowest effective dose
  • Adult dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole twice daily
  • Juvenile dosage based on weight

Administration

  • Do not split, chew, crush or dissolve the tablet
  • Take at least 30 minutes before meals

Interaction

  • Reduction of antihypertensive effect of ACE inhibitors, diuretics & beta-blockers by NSAIDs
  • Increased risk of bleeding complications with warfarin
  • Interference with drug absorption by Esomeprazole
  • Effect of high-fat food on absorption of naproxen

Contraindications

  • Known hypersensitivity to any component
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decreased incidence of gastrointestinal side effects from Naproxen
  • Lower risk of arterial thrombotic events with Naproxen (1000 mg daily)
  • Gastrointestinal adverse events
  • Oedema, hypertension, and cardiac failure reported with NSAID treatment

Pregnancy & Lactation

  • Avoidance during first and second trimester of pregnancy
  • Contraindicated during the third trimester of pregnancy
  • Avoid use during breastfeeding
  • Impairment of female fertility

Precautions & Warnings

  • Avoidance of combination with other NSAIDs
  • Minimize undesirable effects by using lowest effective dose
  • Regular surveillance for long-term treatment
  • Increased frequency of adverse reactions in older people
  • Gastrointestinal, cardiovascular, cerebrovascular, and renal effects

Use in Special Populations

  • Dose adjustment in elderly patients
  • Not recommended for use in patients with moderate to severe renal impairment
  • Close monitoring in patients with mild to moderate hepatic impairment
