Dosage

  • Dosage form: Tablet
  • Dosage strength: 375mg naproxen/20mg esomeprazole, 500mg naproxen/20mg esomeprazole
  • Dosing frequency: Twice daily
  • Administration: 30 minutes before meals

Indications

  • Relief of signs and symptoms of osteoarthritis
  • Relief of signs and symptoms of rheumatoid arthritis
  • Relief of signs and symptoms of ankylosing spondylitis
  • Decrease in the risk of developing gastric ulcers

Pharmacology

  • Consists of an immediate-release esomeprazole magnesium layer and an enteric-coated naproxen core
  • Naproxen inhibits prostaglandin synthesis
  • Esomeprazole suppresses gastric acid secretion

Interaction

  • Concomitant use with ACE inhibitors, diuretics, and beta-blockers may reduce antihypertensive effect
  • Concomitant use with warfarin may increase risk of bleeding complications
  • Inhibition of gastric acid secretion may interfere with absorption of certain drugs (e.g., ketoconazole, iron salts, digoxin)

Contraindications

  • Known hypersensitivity to any component of the tablet or substituted benzimidazoles
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Clinical trials and data suggest a small increased risk of arterial thrombotic events with long-term NSAID use
  • Common adverse events include gastrointestinal issues such as nausea, vomiting, diarrhea, and dyspepsia
  • Risk of hypertension, cardiac failure, peptic ulcers, GI bleeding, and exacerbation of colitis

Pregnancy & Lactation

  • Not recommended during the first and second trimester of pregnancy
  • Contraindicated during the third trimester of pregnancy
  • Not recommended during breastfeeding

Precautions & Warnings

  • Avoid use with other NSAIDs and cyclooxygenase-2 selective inhibitors
  • Consider combination therapy with protective agents for high-risk patients
  • Increased risk of gastrointestinal infections and reduced vitamin B12 absorption

Overdose Effects

  • Symptoms may include lethargy, dizziness, epigastric pain, hypertension, and renal dysfunction
  • No specific antidotes, management is symptomatic and supportive
