Description
- Nebifast is a beta-adrenergic receptor blocking agent indicated for hypertension, essential hypertension, chronic heart failure, and treatment of stable mild and moderate chronic heart failure in elderly patients.
Indications
- Hypertension
- Treatment of essential hypertension
- Chronic heart failure (CHF)
- Treatment of stable mild and moderate chronic heart failure in elderly patients
Pharmacological Properties
- Beta adrenergic receptor blocking agent
- Inhibits both beta1 and beta1 adrenergic receptors
- Lacks intrinsic sympathomimetic and membrane stabilizing activity
- Contributes to decreased heart rate, myocardial contractility, and vasodilation
Pharmacokinetics
- Metabolized by glucuronidation and hydroxylation by CYP2D6
- Mean peak plasma concentration occurs approximately 1.5 to 4 hours post-dosing
- Approximately 98% human plasma protein binding
Absorption and Distribution
- Similar to an oral solution
- Mean peak plasma nebivolol concentrations occur approximately 1.5 to 4 hours post-dosing
- Food does not alter the pharmacokinetics
Metabolism and Excretion
- Predominantly metabolized via direct glucuronidation
- Recovery of 38% to 67% in urine and feces for extensive and poor metabolizers
Drug Interactions
- Myocardial depressants or inhibitors of AV conduction
- Digitalis glycosides
- CYP2D6 Inhibitors (quinidine, propafenone, fluoxetine, paroxetine, etc.)
Dosage & Administration
- Recommended starting dose of 5 mg once daily, with or without food
- Dose can be increased at 2-week intervals up to 40 mg
Contraindications
- Severe bradycardia
- Greater than first degree heart block
- Cardiogenic shock
- Decompensated cardiac failure
- Sick sinus syndrome without a permanent pacemaker
- Severe hepatic impairment
- Hypersensitivity
