Description

  • Nebifast is a beta-adrenergic receptor blocking agent indicated for hypertension, essential hypertension, chronic heart failure, and treatment of stable mild and moderate chronic heart failure in elderly patients.

Pharmacological Properties

  • Beta adrenergic receptor blocking agent
  • Inhibits both beta1 and beta1 adrenergic receptors
  • Lacks intrinsic sympathomimetic and membrane stabilizing activity
  • Contributes to decreased heart rate, myocardial contractility, and vasodilation

Pharmacokinetics

  • Metabolized by glucuronidation and hydroxylation by CYP2D6
  • Mean peak plasma concentration occurs approximately 1.5 to 4 hours post-dosing
  • Approximately 98% human plasma protein binding

Absorption and Distribution

  • Similar to an oral solution
  • Mean peak plasma nebivolol concentrations occur approximately 1.5 to 4 hours post-dosing
  • Food does not alter the pharmacokinetics

Metabolism and Excretion

  • Predominantly metabolized via direct glucuronidation
  • Recovery of 38% to 67% in urine and feces for extensive and poor metabolizers

Drug Interactions

  • Myocardial depressants or inhibitors of AV conduction
  • Digitalis glycosides
  • CYP2D6 Inhibitors (quinidine, propafenone, fluoxetine, paroxetine, etc.)

Dosage & Administration

  • Recommended starting dose of 5 mg once daily, with or without food
  • Dose can be increased at 2-week intervals up to 40 mg

Contraindications

  • Severe bradycardia
  • Greater than first degree heart block
  • Cardiogenic shock
  • Decompensated cardiac failure
  • Sick sinus syndrome without a permanent pacemaker
  • Severe hepatic impairment
  • Hypersensitivity
