Indications

Pharmacology
- Inhibits peristaltic activity
- Binds to opiate receptor in the gut wall
- Reduces propulsive peristalsis
- Increases intestinal transit time
- Increases tone of anal sphincter
- Inhibits electrolyte and fluid secretion
- Orally administered capsule
- Absorption rate more than 65%
- Peak serum levels of 2-3 µgm/litre at 4 hours
- Excreted unchanged in faeces
- Presystemic first-pass metabolism in gut wall and liver
- Elimination in urine, bile, and faeces
Dosage & Administration
- Adult dosage: 2 capsules initially, then 1 capsule after each loose stool
- Maximum daily dose: 8 capsules
- Children (9-12 years): 1-6 capsules daily
- Children (5-9 years): 1-4 capsules daily
- Chronic diarrhea: 2-4 capsules daily for adults, 1-2 capsules daily for children
