Indications
- Hypertension
- Treatment of essential hypertension
- Chronic heart failure (CHF)
- Treatment of stable mild and moderate chronic heart failure in addition to standard therapies in elderly patients
Pharmacology
- β adrenergic receptor blocking agent
- Inhibition of both β1 and β1 adrenergic receptors
- Lack of intrinsic sympathomimetic and membrane stabilizing activity
- Decreased heart rate
- Decreased myocardia contractility
- Diminution of tonic sympathetic outflow to the periphery from cerebral vasomotor centers
- Suppression of renin activity
- Vasodilation and decreased peripheral vascular resistance
Dosage & Administration
- Individualized dose based on patient's needs
- Recommended starting dose of 5 mg once daily
- Dose can be increased at 2-week intervals up to 40 mg
- Administration with or without food
- Not beneficial with more frequent dosing regimen
Interaction
- Careful use with myocardial depressants or inhibitors of AV conduction
- Avoid combination with other β-blockers
- Cautious monitoring with catecholamine-depleting drugs
- Caution when co-administered with CYP2D6 inhibitors
Contraindications
- Severe bradycardia
- Heart block greater than first degree
- Cardiogenic shock
- Decompensated cardiac failure
- Sick sinus syndrome (unless a permanent pacemaker is in place)
- Severe hepatic impairment (Child-Pugh >B)
- Hypersensitivity to any component of the product
Side Effects
- Headache
- Nausea
- Bradycardia
Precautions & Warnings
- Advise against abrupt discontinuation in patients with coronary artery disease
- Caution in patients with compensated congestive heart failure
- Not studied in patients with angina pectoris or recent MI
- Patients with bronchospastic diseases should not receive β-blockers
- Close monitoring needed during anesthesia and major surgery
- Use with caution in patients with diabetes, thyrotoxicosis, peripheral vascular disease, or impaired renal/hepatic function
- Potential increased reactivity to allergens for patients with severe anaphylactic reactions
Use in Special Populations
- Caution in patients with severe renal impairment
- Not studied in patients receiving dialysis
- Caution in patients with moderate hepatic impairment
- Contraindicated in patients with severe hepatic impairment
- Increased reactivity to repeated challenges for patients with a history of severe anaphylactic reactions to allergens
