Indications

  • Hypertension
  • Treatment of essential hypertension
  • Chronic heart failure (CHF)
  • Treatment of stable mild and moderate chronic heart failure in elderly patients.

Pharmacology

  • β adrenergic receptor blocking agent
  • Inhibition of β1 and β1 adrenergic receptors
  • Lack of intrinsic sympathomimetic and membrane stabilizing activity
  • Decreased heart rate
  • Decreased myocardial contractility
  • Diminution of tonic sympathetic outflow to the periphery from cerebral vasomotor centers
  • Suppression of renin activity
  • Vasodilation and decreased peripheral vascular resistance
  • Metabolism by glucuronidation and hydroxylation by CYP2D6
  • Mean peak plasma concentration
  • Plasma protein binding
  • Excretion in urine and feces
  • Interaction with digoxin and warfarin

Dosage & Administration

  • Individualized dose based on patient needs
  • Recommended starting dose of 5 mg once daily
  • Can be administered with or without food
  • Dose increment at 2-week intervals up to 40 mg
  • Unlikely benefit from more frequent dosing regimen

Interaction

  • Caution with myocardial depressants or AV conduction inhibitors
  • Avoid combination with other β-blockers
  • Monitor patients receiving catecholamine-depleting drugs
  • Use caution with CYP2D6 inhibitors (quinidine, propafenone, fluoxetine, paroxetine, etc.)

Contraindications

  • Severe bradycardia
  • Greater than first-degree heart block
  • Cardiogenic shock
  • Decompensated cardiac failure
  • Sick sinus syndrome without a permanent pacemaker
  • Severe hepatic impairment
  • Hypersensitivity to any component of the product

  • Angina and acute myocardial infarction considerations
  • Avoidance in bronchospastic diseases

  • Peripheral vascular disease caution
  • Caution with non-dihydropyridine calcium channel blockers

Use in Special Populations

  • Caution in patients with impaired renal function
  • Caution in patients with moderate hepatic impairment
  • Contraindicated in patients with severe hepatic impairment
  • Risk of anaphylactic reactions in patients with a history of severe reactions to allergens
