Indications
- Severe extensive psoriasis
- Palmo-plantar pustular psoriasis
- Severe congenital ichthyosis
- Severe Darier’s disease (keratosis follicularis)
Pharmacology
- Retinoid
- Aromatic analogue of vitamin A
- Regulation of RNA/DNA synthesis
- Modulation of factors influencing epidermal proliferation
- Modification of glycoprotein synthesis
- Modulation of the immune response
- Modulation of cellular differentiation in the epidermis
Dosage & Administration
- Initial daily dose of 25 mg or 30 mg for 2 to 4 weeks
- Daily dosage of 25-50 mg for 6 to 8 weeks
- Maximum dose of 75 mg/day
- Starting dose of 10 mg for Darier’s disease
- Daily dosage in children about 0.5mg/kg
- Maximum daily dose for children is 35 mg
Interaction
- Formation of etretinate with ethanol intake
- Contraindication with methotrexate, tetracyclines, vitamin A, and other retinoids
- Partially reducing the protein binding of phenytoin
- Inadequate contraception with low dose progesterone-only products (minipills)
- No interaction with coumarin type anticoagulants (warfarin)
Contraindications
- Highly teratogenic
- Contraindicated in pregnant women and women of childbearing potential
- Contraindicated in women who are breastfeeding
- Contraindicated in patients with severe hepatic or renal impairment
- Contraindicated in patients with chronic elevated blood lipid values
- Contraindicated with other retinoids or Vitamin A
- Contraindicated in cases of hypersensitivity to the preparation
Side Effects
- Most patients experience adverse effects
- Dose-related side effects
- Toxic dose is close to therapeutic dose
- Initial worsening of psoriasis symptoms possible
Pregnancy & Lactation
- Contraindicated in pregnant women and nursing mothers
- Contraindicated in women who might become pregnant within 2 years after treatment
Precautions & Warnings
- Exceptionally high risk of giving birth to a deformed child
- Enhanced effects of UV light
- Hepatic function and serum lipids monitoring
- Altered glucose tolerance
- Possibility of alopecia
- Monitoring for decreased night vision
- Immediate discontinuation for severe symptoms
Overdose Effects
- Manifestations of acute Vitamin A toxicity
- Similar signs and symptoms for accidental or deliberate overdosage
- Specific treatment unnecessary due to low acute toxicity
