Indications
- Acute and maintenance treatment of schizophrenia
- Treatment of related psychoses where positive and/or negative symptoms are prominent
- Treatment of acute manic or mixed episodes in bipolar disorder
- Treatment of bipolar disorder with or without psychotic features
- Treatment of bipolar disorder with or without rapid cycling course
Pharmacology
- Antipsychotic agent with affinities for serotonin 5HT2A/2C, 5HT3, 5HT6 receptors
- Affinities for dopamine D1, D2, D3, D4, D5 receptors
- Affinities for cholinergic muscarinic receptors M1-M5
- Affinities for alpha1 adrenergic receptors and histamine H1 receptors
- Mechanism of action is unknown, proposed efficacy through a combination of dopamine and serotonin type 2 (5HT2) antagonism
- Well absorbed after oral administration, peak plasma concentrations within 5 to 8 hours, absorption not affected by food
- Not mutagenic, clastogenic, or carcinogenic
Dosage
- Initial dose of 5-10 mg once daily for schizophrenia and bipolar disorder
- Daily dosage may be adjusted within the range of 5-20 mg daily
- Maximum dose of 20 mg daily
- Initial dose of 15 mg once daily for monotherapy for mania
- Daily dosage may be adjusted within the range of 5-20 mg daily
- Maximum dose of 20 mg daily
Administration
Can be given without regard to meals, gradual tapering of the dose should be considered while discontinuing
Interaction
- May antagonize the effects of levodopa and dopamine agonists
- Drugs that induce CYP1A2 or glucoronyl transferase enzymes may increase clearance
- Inhibitors of CYP1A2 may potentially inhibit elimination
- Carbamazepine may increase clearance
- Concomitant administration of activated charcoal reduces oral bioavailability by 50-60%
- Caution advised with centrally acting drugs and alcohol
Contraindications
- Known hypersensitivity to any ingredient
- Known risk for narrow-angle glaucoma
Side Effects
- Very common undesirable effects include somnolence and weight gain
- Other effects may include increased appetite, elevated glucose and triglyceride levels, dizziness, akathisia, Parkinson's disease, dyskinesia, orthostatic hypotension, anticholinergic effects, asthenia, edema, photosensitivity reaction
Pregnancy & Lactation
- Use in pregnancy only if potential benefits justify potential risk to the fetus
- Patients advised to notify physician if they become pregnant or intend to become pregnant during treatment
- No report to show teratogenicity
- Patients should not breastfeed while taking
Precautions & Warnings
- Use cautiously in patients with history of seizures or conditions associated with seizures
- Use cautiously in patients with low leukocyte and/or neutrophil counts, drug-induced bone marrow depression/toxicity, hypereosinophilic conditions, impaired hepatic function, and those using hepatotoxic medicines and centrally acting drugs
- Caution patients about operating hazardous machinery, including motor vehicles
Use in Special Populations
- Not studied in subjects under 18 years of age
- Starting dose for elderly patients (age 65 and over) is 5 mg/day
- Starting dose for patients with hepatic or renal impairment is 5 mg/day
