Therapeutic Class

  • Angiotensin II receptor blocker

Indications

  • Treatment of hypertension
  • Can be used alone or in combination with other antihypertensive agents

Pharmacology

  • Olmesartan blocks the vasoconstrictor effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT 1 receptor in vascular smooth muscle
  • Has more than a 12,500-fold greater affinity for the AT 1 receptor than for the AT 2 receptor
  • Does not inhibit ACE (kininase II) or affect the response to bradykinin

Dosage & Administration

  • Usual recommended starting dose of Olmesartan is 20 mg once daily when used as monotherapy
  • For patients requiring further reduction in blood pressure after 2 weeks, the dose may be increased to 40 mg
  • No initial dosage adjustment recommended for elderly patients or patients with moderate to marked renal impairment or hepatic dysfunction
  • Paediatric use has not been established

Interaction

  • No significant drug interactions reported in studies with digoxin or warfarin
  • Bioavailability of olmesartan not significantly altered by co-administration of antacids
  • Not metabolized by the cytochrome P450 system and has no effects on P450 enzymes

Contraindications

  • Contraindicated in patients hypersensitive to any component of the product

Side Effects

  • Well tolerated with an incidence of adverse reactions similar to placebo
  • The overall frequency of adverse reactions was not dose related
  • The only adverse reaction that occurred in more than 1% of patients treated with Olmesartan and at a higher incidence versus placebo was dizziness

Pregnancy & Lactation

  • Discontinue use when pregnancy is detected
  • Drugs that act directly on the renin-angiotensin system can cause injury and even death to the developing fetus
  • Potential adverse effects on nursing infant

Precautions & Warnings

  • Changes in renal function may be anticipated in susceptible individuals
  • Treatment may be associated with oliguria, progressive azotemia, and acute renal failure

Overdose Effects

  • No known experience of overdose with Orbas
  • Potential effects of overdosage include hypotension and tachycardia
  • Clinically significant hypotension due to overdose requires active support of the cardiovascular system
