Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Selective blockade of AT 1 receptor in vascular smooth muscle
- Does not inhibit ACE (kininase II)
- Blocks negative regulatory feedback of angiotensin II on renin secretion
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily
- Further reduction in blood pressure after 2 weeks may increase dose to 40 mg
- Doses above 40 mg do not appear to have a greater effect
Interaction
- No significant drug interactions reported with digoxin or warfarin
- Bioavailability not significantly altered by antacids
- No interaction with cytochrome P450 system
Contraindications
- Hypersensitivity to any component
Side Effects
- Well tolerated with incidence of adverse reactions similar to placebo
- Most common adverse reaction: dizziness
Pregnancy & Lactation
- Discontinue drug when pregnancy is detected
- Can cause injury and death to developing fetus when used during second and third trimesters
- Undetermined excretion in human milk
Precautions & Warnings
- May cause changes in renal function
- May be associated with oliguria and/or progressive azotemia
Overdose Effects
- No experience of overdose reported
- Likely effects include hypotension and tachycardia
